Contact and directory information.  Players are responsible to keep the club informed of any changes to this information.

EISD Physical

Physicals must be completed every year. We MUST have a copy of your physical on file. 

For those students who have participated in UIL sports at Westlake this school year, you can get a copy of the physical form in the training room from Brad Hawkins or Vicki Stafko.  You may contact them at 732-9289, ext. 6 or bhawkins@eanesisd.net or vstafko@eanesisd.net .

If the player has not had a 2009-2010 physical, download and complete the form, then have your physician sign.  Please check with Jim Robison or your family physician regarding physicals needed.

Westlake Rugby Liability Waiver 

Download and print.  Both the player and parent need to read, sign and turn in at practice the week of January 4, 2010.  This form has been adapted from other Westlake sports for our club.

USA Liability Waiver - Minor (under the age of 18)

USA Liability Waiver - Adult (18+years)

This form is required by USA Rugby for proper registration of our players and team.  It allows our club to obtain insurance required by field owners.

USA Rugby Registration Form

Each player will need to complete top portion and sign along with a parent at the bottom.  Do not complete the middle section regarding fees.  These are paid by the club for each player and are part of player dues.  All players must be registered by January 16, 2010.  

A&M Tournament Waiver Form

This form is specific to the Gareth Jones Tournament Challenge Cup to be held on the Texas A&M Campus on April 10th. 

Rugby Waiver for State Tournament - not available at this time

Required for Players participating in the TYRA State Tournament.

Photo ID - Photocopy of driver’s license or student ID

Birth certificate – photocopy

Medical Insurance – photocopy each side of card
