Player Dues, Forms and Documents

2012 Season

Registration for the 2012 Season will begin November 2nd, 2011.

Dues for the 2010-2011 Season are $350.00. An additional $35.00 Eanes Participation Fee will be collected if you are not playing another UIL sport at Westlake and have not paid the $250.00 Eanes Athletic Participation Fee. Dues include each player's USA and Texas Rugby registration fees, training shirt, t-shirt, rugby shorts and game socks, tournament registration fees, EISD facilities charges and various team supplies. Additional travel expenses if we are playing out of town and there is a hotel stay.Post season tournaments will require travel expenses for those participating. This year USA will be providing Insurance coverage for all players that are enrolled. Because of USA Rugby Rules and Insurance Coverage, DUES must be paid prior to any type of contact drills so that players can be registered and covered with USA Rugby.

The following forms and documents are required for each Westlake Rugby player:

Dues	Players that play other UIL Westlake Sports- $350 Non-UIL Players- $350 + $35 Athletic Part. fee= $385.00 Dues may be payed online with PayPal
WRC Player Registration Form	Players are responsible to keep the club informed of any changes to this information.
Eanes Physical and Medical History Forms	Physicals are to be completed each year. We MUST have a copy of your physical on file. For those students who have participated in UIL sports at Westlake this school year, you can get a copy of the physical form in the WHS training room. If the player has not had a 2011-12 physical, download and complete the form and have it signed by your physician.
WRC Acceptance of Requirements	Form signed by player and parent accepting the guidelines of behavior set by the Westlake HS Code of Conduct
WHS Release of Student Info	The Release of Information form is required so that high school eligibility can be provided to USA Rugby before players begin match play.
WRC Release of Liability Waiver WRC Release of Liability Waiver-U15	Both the player and parent need to read, sign and turn in before participating in any regular season practice. Note: Freshman Use Under 15 Form
USA Rugby Agreement for U15 Player to Play High School or U19 Rugby	FRESHMAN ONLY: USA Rugby Form to be filled out by all players that are 15 and under on or before Sept.1st, 2011. (Freshman)
USA Rugby (MINOR) Individual Enrollment Application	This form is required by USA Rugby. It allows our club to be certified for cup play and tournaments. Registration of our team members is necessary for club insurance required by field owners. Each player will need to complete the top portion and sign along with a parent if under 18 (Minor). Use the Adult form if player is 18 years of age. Do not complete the middle section regarding fees. These are paid by the club as they are part of player dues.
